Discharge Planning and How to Find and Use Public Services: Training for Clinicians
This module reviews the rights that people with disability have to be supported in the least restrictive setting, along with legal and family challenges that are unique to the non-geriatric population. In addition, the brain injury service delivery system is described, and public resources available to maximize the chance for community discharge and long-term community support resources are outlined. People of all ages are transferred to nursing facilities from hospitals when brain injury is severe enough to require a longer stay for medical and rehabilitative recovery. Regardless of the level of ongoing disability, people have goals for return to home/community. The primary target audience for this training includes nursing home administrators, case managers, social workers, and discharge planners.
